-pt.หญิง trauma มา ระวัง! pregnancy ห้าม CT, film
-investigation –> เริ่มจาก non invasive ก่อน
-สงสัย pregnancy ทำไงนะ?? ส่ง pregnancy test เลย
-ไม่ว่าเพศไหนอายุก็สำคัญนะ ระวัง! โรคประจำตัว/conpensate ต่อ blood loss ได้น้อยกว่า/CHF ใน elderly when load volume
-Stab wound ดู Size, depth
-Blast injury มีพลังงานจลน์ = 1/2mv*v –> ความเร็วมีผล
-Shot gun คือลูกซอง มีหลายนัด Gun shot คือลูกโทน มีนัดเดียว
-High-velocity missile = ความเร็วของลูกกระสุนมากกว่า 2000-2500 ฟุต/s เช่น ปืน m16, rifle, 9mm,11mm
ดังนั้น pt trauma มา ทำไงนะ???
-ดู v/s
-ABCD
Airway – ให้ pt เปล่งเสียง เล่าเรื่องดู ถ้าได้ถือว่าเยี่ยมเลย
- พูดไมได้/unconscious –>open airway
- ! cervical spine ไม่ใส่ philadephia collar 2 ข้อ คือ 1.pt รู้สึกตัวดี 2.ไม่มี neurodeficit นอกนั้นใส่หมด
Breathing – ดู Rate
- rhythm
- breath sound
- chest movement
- คลำ trachea shift –> ใช้นิ้วชี้กับนิ้วนาง fix ที่ head of clavicle นิ้วกลางคลำ ถ้า shift –> tension pneumothorax–> thoracocenthesis เพื่อระบายลมออกบางส่วน (ใช้เข็มเบอร์ 18-16 -> medicut เจาะที่ 2nd ICS at MCL) –> ทำ ICD ถ้าเป็น hemothorax ใช้เข็มเบอร์ 36 ถ้า pnuemothorax เข็มเบอร์ 32,28 ในเด็กใช้ ng tube แทน เจาะที่ขอบบนของribล่าง ICS 4th-5th anterior to MAL ใส่ให้รูสุดท้าย (sentinel) อยู่ใน pleura สอดเข้าไปทิศ posterosuperior –> x-ray and เย็บผูกให้แน่น –> อย่าลืมถอดเข็ม thoraco ออก
- ET tube – ช: No.8
- ญ: No 7.5
- ฝรั่ง: No.9
- blow cuff ไม่ให้ air leak
- practically 3 +/- 2 เหนือ carina
- ใส่ไปที่ 20 –> blow cuff + ventilate
- แล้วฟัง chest –> มีแนวโน้มไปข้างขวามากกว่า ฟัง rt ก่อนได้ยิน breath sound แน่ใจได้แล้วว่าไม่ไป esophagus แน่นอน ฟัง lt –> decrease breath sound ให้ลองขยับดู เวลาฟัง lung ฟัง 5 จุด RUL/LUL/RLL/LLL/epigastrium
Circulation -> Bleeding
-bimanual compression
-ไม่แนะนำ blind clamp
-tourniquet ใช้กรณี traumatic amputation
-ดู bleed 3 จุดหลัก
1.chest -> cxr -> massive hemothorax -> OR -> internal mammillary a. (parasternum)
2.Abd
3.Pelvis -> bleed จาก vein, bone ครึ่งๆ เลย artery~10%เท่านั้น ก็แก้โดย reduce pelvis volume ให้ space เหลือน้อย เกิด temponade effect เป็นการห้ามเลือดไปในตัว
-Beck’s triad –> cardiac temponade
1.distance heart sound
2.BP drop
3.JVP engorgement
